2. RN/PT/OT/CNA/LPN/ HHA Certification /License (Front/Back)
3. Driver's license (Front and Back Copy)
4. BLS, ACLS  (Front and Back Copy)
5. Social Security Card  (Front and Back Copy)
6. Less than 60 days Physical Exam
7. Less than 60 days  - Chest X-ray or PPD test
8. A 10 panel drug test
9. MMR, Varicella, Hep B, Hep C & Flu shot or Titers
10. Mask fit test (Alpha protech or 3M 1860)
11. Fire card (for LA County hospitals)
12. W-4 Form
13. I-9 Form
14. 3 Employment Reference Check
